Workplace deaths as reported by OSHA

Saw this on another forum and thought I would pass it along.  From 8-1-12 to 5-24-14I went through the first half dozen pages or so and noticed a few deaths were from repairs on petroleum tanks....https://www.osha.gov/dep/fatcat/fy14..._summaries.pdf

Reply:I check the OSHA report once or twice a year.  I think it helps ground me when it comes to doing things safely.  The one take away I have from the report is to always think twice if I'm doing one of the following:Working on a ladderWorking around or with a forkliftWorking in  a scissors or boom liftWorking in a ditch or excavation deeper than my waistMore people get killed by one of these 4 things than anything else.  The other conclusion I came to was I'll never work for a company doing "landscaping", tree cutting, or lumbjack work.  Entirely too dangerous compared to any other profession or way to make a living.Benson's Mobile Welding - Dayton, OH metro area - AWS Certified Welding Inspector

Reply:Wow!!! That is just a horrible list. There was one on 1/31/2014, Leavitt Tube. My buddy plows that facility, and I happen to have been plowing there with him that night. I showed up right as all the police were leaving. I saw the machine that got him. It had me pretty messed up for a while. Nobody should not come home to their family, from work. They said he was a master at the machine, but forgot to take it off auto. The machine moved as fast as it could. When he went to clear the jam, he did it really fast which told the machine to advance really fast, and basically tore him in half."Where's Stick man????????" - 7A749"SHHHHHH!! Reply:I've been in the unfortunate position of being involved in a few workplace deaths, either as part of the emergency response, or part of the investigation team.  The thing that comes out time and time... and time again is that most of them were not only preventable, but in some ways predictable because there were clear warning signs there (that unfortunately weren't picked up on).  You usually don't get killed by the first lightning bolt of a storm that comes from nowhere - bolts will be hitting the ground as the storm approaches, and it is the same in the workplace - there will be several "near misses" (that unfortunately either don't get reported to management or don't get acted upon) before the fatal incident.  It's not just occupational safety that is like this, process safety in refineries and chemical plants is the same - the warning signs are there, although usually in process safety the management get reports of the lead up smaller incidents but either don't act on them or fail to recognise what the signs are telling them - BP Texas City was a classic (almost textbook), and totally preventable example.

Reply:Worker crushed by one-ton object that fell on him while welding.Damn, that must suck big time - the poor guy probably never even knew what happened.
Reply:Unfortunately the list seems incomplete. It doesn't cover the 101 firefighters that died in the line of duty in 2013 nor the 104 police officers that died in the line of duty in that same year.Miller Multimatic 255
Reply:There was one yesterday in our paper's national news that I would hate to see.  Worker was killed when he was sucked into a chipper while doing tree work. Not much left of him I would Imagine.

Reply:Engineers used to calculate the number of deaths there will be on a large construction project.  Nowadays that can be expensive.  Employers only wake up when they are faced with crippling increased insurance rates.  Only then do they fall in line and enforce things like fall protection.  Many years ago I was told by one compensation inspector that roofing contractors had to pay the Sate of Florida 100 dollars a day for each man working on a roof.  At the time locally we paid 28 Canadian dollars a day for each faller using a chainsaw taking down trees.  He told me what it was for helicopter logging pilots.. It was an astronomical amount.  The reason being that crashes were frequent and the pilot was making over 100 K a year.  His wife and children would be paid a survivor's pension that was representative of his income.
