Any concrete guys want to weigh in on what happened here?

Reply:Your "threaded reinforcing" is a bit too close to the outside of concrete for most codes... I'm a bit surprised to see the  base pad sitting on the floor at the bottom. What did they do pour the column base on the slab, or did they hang the scaffolding foot out over the floor cutout?.No government ever voluntarily reduces itself in size. Reply:You can even see the square foot on the scaffolding Jack sticking out at the floor level. Lots of things get left in concrete before a pour. I have a friend who used to work building power plants in Poland. They were removing forms after a concrete pour and found a nice rubber boot stuck in the concrete. Boots were hard to come by so they began chipping it out and found that someone was still wearing it. It was a hard life over there.
