Can I get a replacement CWB Welder card?

I lost one of my "Welder Qualification" CWB cards with my FCAW/MCAW ticket information.  Does anyone know if it is possible to get a replacement?

Reply:A good friend of mine got his all position smaw ticket and the card only listed 3 positions instead of the 4 positions he had passed.
